NO SHOW POLICY
ANY PATIENT THAT MAKES AN APPOINTMENT & DOES NOT SHOW TO THIS APPOINTMENT WILL INCUR A $50 NO SHOW FEE THAT WILL BE CHARGED TO THE PATIENT’S ACCOUNT. THE NO SHOW FEE WILL BE DUE BEFORE ANY TREATMENT CAN BE DONE AND/OR BEFORE PATIENT CAN BE SEEN AGAIN.
FINANCIAL POLICY
IF PATIENT’S INSURANCE DOES NOT PAY FOR ANY PROCEDURE, THE PATIENT IS RESPONSIBLE FOR THE BALANCE & THE BALANCE MUST BE PAID BEFORE PATIENT CAN BE SEEN AGAIN.
CASH/INSURANCE PATIENTS
PAYMENT IS DUE BEFORE SERVICES ARE PERFORMED, THIS IS NOT NEGOTIABLE. THIS MEANS BEFORE YOU GO IN FOR EXAM/X-RAYS/TREATMENT, YOU WILL BE REQUIRED TO PAY.
NITROUS PATIENTS/CROWN & BRIDGE PATIENTS
IF YOU MAKE AN APPOINTMENT & YOU DO NOT SHOW, YOU WILL INCUR A CHARGE OF $50 FOR THAT APPOINTMENT & THIS WILL NOT BE APPLIED TO YOUR TREATMENT. FOR THESE PROCEDURES, 1-2 HOURS IS BLOCKED ON THE SCHEDULE ESPECIALLY FOR YOU.
IF YOU RESCHEDULE THE APPOINTMENT THE DAY OF THE APPOINTMENT, YOU WILL STILL INCUR THIS $50 CHARGE BECAUSE THIS IS NOT ENOUGH TIME TO PUT SOMEONE ELSE IN THAT APPOINTMENT SLOT.
IF APPOINTMENTS ARE RESCHEDULED DUE TO INCLEMENT WEATHER, EMERGENCIES AND/OR OFFICE EMERGENCIES THEN NO FEE WILL BE INCURRED.
